Social Care Reviews

SC1       Care Act Assessment

To assess an individual’s needs for care and support and determine their eligibility for services under the Care Act 2014. It ensures that the appropriate care and support are provided to enhance their quality of life. The legislation on this can be viewed at Care Act 2014: Section 9.

SC2       Care Review

To periodically review an individual’s care plan to ensure that their needs are being met and make necessary adjustments to their care and support arrangements. Statutory guidance on the completion of care reviews can be viewed at Care and Support Statutory Guidance

SC3       Annual Contract Review

Where the Local Authority assess and review if the placement and care plan are appropriate for the individual to remain living in the care home. Typically Local Authority contracts require a review to take place before the expiry of an existing contract, and then it is the responsibility of the Local Authority to arrange subsequent contract reviews in line with it’s own minimum requirements.

SC4       Mental Capacity Assessment (MCA)

To assess an individual’s capacity to make specific decisions for themselves at a particular time. It ensures that decisions are made in the person’s best interests if they lack capacity. Guidance and legislation around this can be viewed at NHS MCA Guidance and MCA 2005 Legislation

SC5       Best Interests/DOLS Assessment

To assess whether it is in a person’s best interests to be deprived of their liberty for care and treatment purposes, and to ensure that the deprivation is necessary and proportionate. SC6       Relevant Person’s Representative (RPR) Visit

To ensure the rights of individuals under a Deprivation of Liberty Safeguards (DOLS) authorization are protected by having a representative visit and support them regularly. Guidance and legislation on these visits can be viewed at DoH RPR guidance and RPR legislation

SC7       Enhanced Care Review

To conduct a detailed review of an individual’s care plan and services on an enhanced care basis, ensuring their specific needs and preferences are being met. Health Care Reviews

HC1      Notification of Admission Assessment (NOA)

NHS Somerset ICB need to complete their initial nursing assessment for new nursing admission in order for the Care Home to be paid FNC.

HC2      Community Nursing Assessment

An internal assessment of nursing needs for those on Residential placements completed between the clinical team at Evolve along with Primary Care and/or District Nurses. HC3      CHC Checklist

To screen individuals to see if they may be eligible for a full CHC assessment. It serves as an initial step to identify those who require further evaluation for Continuing Healthcare. The national framework and assessment tool can be viewed by clicking CHC Checklist guidance and DST Support tool.

HC4      Continuing Healthcare (CHC) Assessment

To determine whether an individual is eligible for NHS Continuing Healthcare, a package of care arranged and funded solely by the NHS for individuals with significant ongoing health care needs. The national framework can be viewed by clicking CHC National Framework

HC5      CHC Review

To regularly review an individual’s eligibility for Continuing Healthcare to ensure they continue to receive the appropriate level of care based on their current health needs. The national framework can be viewed by clicking CHC National Framework

HC6      Fast-track CHC

HC7      Funded Nursing Care (FNC) Assessment

To determine if an individual qualifies for Funded Nursing Care, which is a payment made by the NHS to cover the nursing care component of a resident’s care in a care home. The FNC practice guidance is available here FNC practice guidance
